When you call to schedule your first appointment with us (Hooray! Great decision!), please have your insurance cards and policy numbers available so that we may contact your insurance companies to pre-certify your benefits prior to your arrival. Please bring with you the following:

• Physician’s prescription/referral (if required by your insurance company – Note: Medicare patients always require a physician’s prescription)
• Current printed MRI, CT or x-ray reports
• Current insurance card(s)
• Current medication list
• Comfortable clothing allowing for mobility and access to area requiring treatment
• Workers compensation or motor vehicle, please bring the following: claim number, claim adjuster’s name, address, phone and fax number
• Payment for co-pay, co-insurance, self-pay (payment must be made at time services are rendered).
